RN Case Manager
This position will be considered for a full-time RN with certified home care experience. Primary responsibilities include case management of patients discharged from the hospital or rehab setting to their home with certified, skilled nursing services. Provide education of patients and families to promote patient success. Work as part of a small interdisciplinary team environment in the coordination of patient services.
PT - Physical Therapist Case Manager
Responsible for agency admissions, evaluations and case management working with the interdisciplinary team. Communicate with the team and physicians for patient care. Provide goal directed, activity oriented treatment to promote optimum independence in functional mobility of daily life tasks in the patient’s home environment.
RN Clinical Manager
Responsible for direct, daily supervision of professional and para-professional clinical staff assigned to a Patient Care Team and other staff as assigned. Responsible for maintaining quality of patient care in accordance with agency’s objectives and policies, and for planning, coordinating, implementing and evaluating patient care services.
Certified Home Health Aide - CHHA
The Certified Home Health Aide is an integral part of the interdisciplinary team caring for patients in the home. The CHHA works closely with Case Managers and other team members on the assigned Patient Care Team, conducting home health aide visits as assigned by the Clinical Scheduler.
OTR/L - Occupational Therapist
This Occupational Therapist position allows you flexibility of schedule in providing services as part of an interdisciplinary team to patients in their home who have impairments, functional limitations or disease.
Patient Care Team Nurse
The Patient Care Team Nurse is an integral part of the interdisciplinary team caring for patients in the home. The RN Team Nurse assists in the management of patient cases, working closely with RN Case Managers and other team members on the assigned Patient Care Team, conducting home visits as assigned by the Clinical Manager, RN Case Manager and/or Clinical Scheduler. . .
Physical Therapist - Per Diem
Responsible for agency admissions, evaluations and follow-up visits scheduled during the weekend to patients in their residence. Initiate a Plan of Care and appropriate rehabilitative procedures and coordination of services. Communicate with Case Managers and covering physicians for patient concerns.
